Abstract:

The aim of this deliverable is described outcomes from WP2-PERSIST specifications and methodology subtask 2.2 State-of-the-art in cancer survivorship and rehabilitation best practices in follow-up visits. Review policy recommendations for the implementation of care plans in EU member states with a focus on the countries and specific health care organizations where clinical studies will be developed. Review the state of the art in cancer survivor follow-up with a focus on integrated pathways implementation and access to rehabilitation and psychosocial care and methodologies for the assessment of lifestyle factors.
